REGISTRATION FORM INSTRUCTIONS

1.      Please fill in the form completely,

o       include emergency numbers

o       please put your e-mail address; this has been a wonderful way to communicate any updates or changes

o       for each child put the Religious Education Level for the 2010/2011 school year

o       for Religious Education Levels1-5 indicate the session you would like your child to attend next year (Tuesday 4:15 or Wednesday 6:30).

2.      The class schedule will remain the same as this year:

• Tuesday 4:15 – 5:30             Kindergarten, Religious Education Levels 1-5

• Tuesday 6:30 – 8:00             Confirmation Levels 1 and 2

• Tuesday 6:30 – 8:00             Confirmation Level 3 (twice each month)

• Wednesday 6:30 -7:45         Religious Education Levels 1-5

• Sunday during 9:30am Mass           Pre-School

Children’s Liturgy of the Word (no registration necessary)

3.      You can return the completed Registration Form and Fee (please do not delay sending in the registration form if you cannot pay the fee at this time; you can pay the fee at a later date, but it is important to get the registration form by May 23) by:

o       Sending it in with your child when he/she comes to CCD

o       Mailing it to the church office (address above) marked to the attention of Maureen Curtis

o       Placing it in the box in the CCD Hall after Mass

4.      You can also register on-line at www.stedwardbowie.org

5.      The fee for the 2010/2011 School Year will be $30.00 per student with a $60.00 family maximum.  If you are currently enrolled in the program (2009/2010 school year), late fees will apply after May 23.  The fee will then be $45.00 per student with a $75.00 family maximum.

6.      All children who are not attending Catholic Elementary School should be enrolled in the Parish Religious Education Program (commonly called CCD).  Children who are registering for Religious Education Level 2 must have completed Religious Education Level 1, either at a Catholic School or Parish Religious Education Program.  Children registering for the Confirmation Level 3 must have completed Confirmation Levels 1 and 2 either at a Catholic School or Parish Religious Education Program. Children registering for Kindergarten must be 5 by September 1, 2010.
